Special Education Services - Survey

Please read this important message if your student receives Special Education Services from Findlay City Schools:

The Individuals with Disabilities Education Act (IDEA) requires that school districts collect parent involvement survey data for their students with IEPs at least once every six years.  Findlay City Schools is participating in this state-wide survey this spring, between March 1 and April 30, 2021.

The survey, coordinated by The Ohio State University, collects important information that can improve family engagement and services for students with disabilities in schools throughout Ohio.  The survey is short and all participants will be entered into a drawing for several $50 Amazon gift cards.

In order to increase participation from diverse families, the survey invitation has a live link or QR code to access the survey in English, Spanish, Arabic, Chinese, Nepali, and Somali.

Click Here to take the survey in English.  To change the survey from English to another language, click on the box on the upper right corner of the survey.

On behalf of Findlay City Schools, thank you for taking time to complete the survey.
